Keramidit vaikuttavat maksan insuliiniresistenssin kehittymiseen

New evidence for the role of ceramide in the development of hepatic insulin resistance.

TAUSTA ja TARKOITUS: On harvoja ja ristiriitaisia tietoja solunsisäisten sfingolipidien ja erityisesti keramidien liikakertymistä ja korrelaatiosta maksan insuliiniresistenssiin. Tässä tutkimuksessa mitattiin kertyneiten sfingolipidien fraktioita ja selviteltiin maksan insuliiniresistenssin kehittymistä kuten rasvahappojen ja keramidien kuljettajien osuutta tässä prosessissa.

MENETELMÄT: Koe-eläimen maksasolut altistettiin C16:0 ( palmitiinihapolle) lyhyessä ja pitkässä inkubaatiossa ja mitattiin solun sisäiset ja solun ulkopuoliset sfingolipidifraktiot. Palmitiinihapon (C16:0) aiheuttama insuliiniresistenssi mitattiin insuliinitien proteiinien fosforyloitumisen vaihteluista ja erilaisten kuljettajien ilmentymisten vaihteluista.
ENGL.:METHODS: In culture of primary rat hepatocytes, exposed to high concentration of palmitic acid (0.75 mM) during short and prolonged incubation, high performance liquid chromatography was used to assess intra- and extracellular sphingolipid fractions content. Degree of palmitate-induced insulin resistance was estimated by measuring changes in phosphorylation of insulin pathway proteins by western blotting as well as changes in expression of different type of transporters.
TULOKSET. Palmitiinihapon lyhyt ja pitkäaikainen altistus primäärisissä maksasoluissa johti keramidien solunsisäiseen akkumulatioon, mikä esti insuliinin signaloitumistietä. Oli havaittavissa merkitsevä rasvahapon kuljettajaproteiinien (FATP) ja keramidien siirtäjäproteiinien (CERT) expressoitumisen lisääntyminen, mikä oli yhdenmukaista sille ilmiölle, että keramidipitoisuudet kasvoivat solun sisällä. Solunulkoistenkin keramidien pitoisuudet kohosivat miltei kolminkertaisiksi lyhyen inkubaatiojakson jälkeen ja pitkässä inkubaatiossa kaksinkertaiseksi. Palmitaatin lyhyen inkubaation jälkeen nousi merkitsevästi sekä mikrosomaalisen kuljettajaproteiinin (MTP) että ATP:tä sitovan setin kuljettajan ( ABCA1) expressiot .
RESULTS: In our study short and prolonged exposure of primary hepatocytes to palmitic acid resulted in increased intracellular accumulation of ceramide which inhibited insulin signaling pathway. We observed a significant increase in the expression of fatty-acid transport protein (FATP2) and ceramide transfer protein (CERT) what is consistent with enhanced intracellular ceramide content. The content of extracellular ceramide was increased nearly threefold after short and twofold after long incubation period. Expression of microsomal triglyceride transfer protein (MTP) and ATP-binding cassette transporter (ABCA1) was increased significantly mainly after short palmitate incubation.
JOHTOPÄÄTÖS: Saadut tiedot osoittavat, että solunsisäisten keramidien pitoisuuden nousu vaikuttaa osaltaan maksaperäisen insuliiniresistenssin kehittymiseen. Tutkijat esittävät johtopäätöksenään, että rasvahappojen kuljettajaproteiineilla on tässä olennainen osuus, kuten sisäänvirtausta kiihdyttävällä kuljettajalla (FATP2), keramidien siirtäjällä (CERT) ja MIT ja ABCA1 kuljettajaporteiineilla.
CONCLUSION: Our data showed that increase in intarcellular ceramide content contributes to the development of hepatic insulin resistance. We suggest pivotal role of transporters in facilitating fatty acid influx (FATP2), accumulation of ceramides (CERT) and export to the media (MTP and ABCA1).

Metabolinen oireyhtymä ja plasman keramidit
ENGL.: BACKGROUND: Metabolic syndrome (MetS) appears closely linked with ceramide accumulation inducing insulin resistance and toxicity to multiple cell types. Animal studies demonstrate that thiazolidinediones (TZDs) reduce ceramide concentrations in plasma and skeletal muscle and support lowering of ceramide levels as a potential mediator of TZDs' mechanism of action in reducing insulin resistance; however, studies in humans have yet to be reported. This study investigated the effects of pioglitazone therapy on plasma ceramides to understand the mechanism by which TZDs improve insulin resistance in MetS.
ENGL: METHODS: 37 subjects with MetS were studied in a single-center, randomized, double-blind, placebo-controlled, trial comparing pioglitazone to placebo. Data were collected at baseline and after 6 months of therapy. The primary endpoint was the change from baseline in plasma ceramide concentrations.
RESULTS: Treatment with pioglitazone for 6 months, compared to placebo, significantly reduced multiple plasma ceramide concentrations: C18:0 (p = .001), C20:0 (p = .0004), C24:1 (p = .009), dihydroceramide C18:0 (p = .005), dihydroceramide C24:1 (p = .004), lactosylceramide C16:0 (p = .02) and the hexosylceramides C16:0 (p = .0003), C18:0 (p = 0.00001), C22:0 (p = .00002) and C24:1 (p = .0006). Additionally, significant reductions were found when ceramides were grouped by species: ceramides (p = .03), dihydroceramides (p = .02), hexosylceramides (p = .00001), and lactosylceramides (p = .02). The total of all measured ceramides was also significantly reduced (p = .001). Following treatment with pioglitazone, the decrease in some ceramide species correlated negatively with the change in insulin sensitivity (dihydroceramide C16:0, r = -.54; p = .02), and positively with total (lactosylceramide C24:0, r = .53; p = .02) and high molecular weight (lactosylceramide C24:0, r = .48; p = .05) adiponectin measurements; however, significant associations with changes in liver fat and glycemic control reduction were not found.
CONCLUSIONS: Pioglitazone in individuals with MetS induces a potent decrease in plasma ceramides, and some of the changes correlate with changes in insulin resistance and adiponectin levels. This article is protected by copyright. All rights reserved.

Effect of pioglitazone on plasma ceramides in adults with metabolic syndrome.

TAUSTA ja TARKOITUS. Metabolinen oireyhtymä näyttää linkkiytyvän keramidien kertymiseen ja siitä indusoituu insuliiniresistenssiä ja toksisuutta moniin solutyyppeihin. Eläinkokeet osoittavat, että tiatsolidinidionit (TZD) alentavat keramidien pitoisuuksia plasmassa ja luustolihaksissa ja ja tukevat käsitystä, että keramidien pitoisuuksien alentaminen on TZD preparaattien vaikutuksen mahdollinen välittäjä insuliini-resistenssin alentamisvaikutuksessa. Kuitenkaan asiaa ei ole ihmistutkimuksia ei ole vielä raportoitu. Tässä tutkimuksessa katsotaan pioglitatzoniterapian vaikutusta plasman keramideihin, jotta saadaan käsitystä TZD- preparaateistä, millä mekanismilla ne pystyvät parantamaan insuliiniresistenssiä metabolisessa oireyhtymässä.
MENETELMÄ: Kaksoissokkokoe, johon osallistui 37 metabolista oireyhtymää potevaa. Tutkimuksessa verrattiin pioglitazonia placeboon. Kuuden kuukauden terapian jälkeen kerättiin tiedot. primäärisenä lopputuloksena etsittiin muutosta plasman keramideissa.
TULOKSET: K kuuden kuukauden pioglitatzoni terapia alensi merkitsevästi plasman keramideja: Mitatut keramidit olivat C18:0, C20:0, C24:1, Dihydrokeramidit C18:0, C24: 1, LacCer ( laktosylkeramidi) C16:0 ja HexCer ( hexosyylikeramidit C16:0, C18:0, C22:0 ja C24:1 . Lisäksi havaittiin merkitsevä alenema kun ryhmiteltiin eri lajit: keramidit, dihydrokeramidit, HexCer ja LacCer ( glykosfingolipidit) . kaikenkaikkiaan mitatut keramidit alenivat merkitsevästi . Pioglitatzoni terapiasta seuraasi korrelaatiota muutamissa keramidilajeissa: negatiivinen korrelaatio insuliiniherkkyyden muutoksessa ( dihydrokeramidi C16:0) ja positiivinen korrelaatio totaali (laktosylkeramidien C24:0) ja HMW (laktosylkeramidi C24:0) adiponektiinimittauksiin. Kuitenkaan ei havaittu merkitseviä muutoksia maksan rasvasssa eikä glykemisessä kontrollissa.
YHTEENVETO. Pioglitazoni saa aikaan metabolista oireyhtymää potevilla vahvaa alenemistä plasman keramideissa ja jotkut muutokset korreloivat muutoksiin insuliiniresistenssissä ja adiponektiinin pitoisuuksissa.

Tiivistelmä.
Tässä tutkimuksessa selvitellään kalorien vähentämisen vaikutusta tahdonalaisen lihaksiston sfingolipidien aineenvaihduntaan ja osuutta insuliinivaikutuksen kohentajana koe-eläimissä 6 kk kestoisella dieetillä. Toisena dieettinä oli ad libitum (AL, vapaasti mielen mukaan) ja toisessa oli 30%:n kalorireduktio (CR).
Tässä tutkimuksessa selvitellään kalorien vähentämisen vaikutusta tahdonalaisen lihaksiston sfingolipidien aineenvaihduntaan ja osuutta insuliinivaikutuksen kohentajana koe-eläimissä 6 kk kestoisella dieetillä. Toisena dieettinä oli ad libitum (AL, vapaasti mielen mukaan) ja toisessa oli 30%:n kalorireduktio (CR).
Monitoroitiin kehon painoa ja insuliiniherkkyyttä. Kuuden kuukauden päästä mitattiin erään lihaksen avulla insuliinisignalointi ja lipidikirjo. Reduktiodieetti vähensi merkitsevästi insuliinin ja glukoosin pitoisuuksia ja muunsi myös niiden proteiinien ilmenemää ja aktiivisuutta, jotka osallistuvat sfingolipidien muodostamiseen ja aineenvaihduntaan.
Keramidien pitoisuudet lisääntyivät merkitsevästi kalorireduktiossa, kun taas keramidiaineenvaihdunnan tuotteet ( glykosfingolipidit,, HexCer ja LacCer) merkitsevästi vähenivät. Keramidifosfaatit, sfingomyeliini,,sfingosiini (So) ja sfingosiinifosfaatti (S-1-P) eivät eronneet merkitsevästi AL ja CR- ryhmien kesken.
LacCer- määrät korreloivat merkitsevästi surrogaattimerkitsijän tavoin insuliiniresistenssiin.
Keramidiaineenvaihdunnan ne tuotteet (sfingolipidit), joiden tiedetään vaikuttavan insuliinin signalointiin kohonneissa pitoisuuksissaan, olivat merkitsevästi vähentyneet luustolihaksesta kalorireduktiossa olleilla koe-eläimillä. Insuliiniherkkyyden lisääntyminen assosioituu glykosfingolipidipitoisuuksiin.
- ENGL.:This study sought to investigate the effect of calorie restriction (CR) on skeletal muscle sphingolipid metabolism and its contribution to improved insulin action in rats after a 6-month feeding study. Twenty nine (29) male Fischer 344 rats were randomized to an ad libitum (AL) diet or 30% CR. Dietary intake, body weight and insulin sensitivity were monitored. After 6 months, skeletal muscle (vastus lateralis) was obtained for insulin signaling and lipid profiling. CR significantly decreased insulin and glucose levels and also altered the expression and activity of proteins involved in sphingolipid formation and metabolism.
- The quantities of ceramides significantly increased in CR animals (P<.05; n=14-15), while ceramide metabolism products (i.e., glycosphingolipids: hexosylceramides and lactosylceramides) significantly decreased (P<.05; n=14-15).
- Ceramide phosphates, sphingomyelins, sphingosine and sphingosine phosphate were not significantly different between AL and CR groups (P=ns; n=14-15).
- Lactosylceramide quantities correlated significantly with surrogate markers of insulin resistance (homeostasis model of assessment on insulin resistance) (r=0.7; P<.005). Products of ceramide metabolism (glycosphingolipids), known to interfere with insulin signaling at elevated levels, were significantly reduced in the skeletal muscle of CR animals. The increase in insulin sensitivity is associated with glycosphingolipid levels.

Sphingosine 1-phosphate signaling pathway in inner ear biology. New therapeutic strategies for hearing loss?
Abstract
Hearing
loss is one of the most prevalent conditions around the world, in
particular among people over 60 years old. Thus, an increase of this
affection is predicted as result of the aging process in our population.
In this context, it is important to further explore the function of
molecular targets involved in the biology of inner ear sensory cells to
better individuate new candidates for therapeutic application. One of
the main causes of deafness resides into the premature death of hair
cells and auditory neurons. In this regard, neurotrophins and growth
factors such as insulin
like growth factor are known to be beneficial by favoring the survival
of these cells. An elevated number of published data in the last 20
years have individuated sphingolipids not only as structural components
of biological membranes but also as critical regulators of key
biological processes, including cell survival. Ceramide,
formed by catabolism of sphingomyelin (SM) and other complex
sphingolipids, is a strong inducer of apoptotic pathway, whereas
sphingosine 1-phosphate (S1P), generated by cleavage of ceramide
to sphingosine and phosphorylation catalyzed by two distinct
sphingosine kinase (SK) enzymes, stimulates cell survival. Interestingly
S1P, by acting as intracellular mediator or as ligand of a family of
five distinct S1P receptors (S1P1-S1P5), is a very powerful bioactive
sphingolipid, capable of triggering also other diverse cellular
responses such as cell migration, proliferation and differentiation, and
is critically involved in the development and homeostasis of several
organs and tissues. Although new interesting data have become available,
the information on S1P pathway and other sphingolipids in the biology
of the inner ear is limited. Nonetheless, there are several lines of
evidence implicating these signaling molecules during neurogenesis in
other cell populations. In this review, we discuss the role of S1P
during inner ear development, also as guidance for future studies.

AIHE: Tarkoituksena oli katsoa reenauksen vaikutusta insuliiniherkkyyteen ja lihavuudellesekä 2- tyypin diabetekselle tyypilliset keramidit
ENGL.: OBJECTIVE: To assess the effect of exercise training on insulin sensitivity and plasma ceramides in obesity and type 2 diabetes (T2D).
MENETELMÄT: 24 aikuista joilla oli lihavuutta ja normaali glukoositoleranssi (n= 14) tai diabetes ( n= 10) tutkittiin ennen testiä ja 12 viikkoa treenausohjelman jälkeen. Treenaus oli 5 kertaa viikossa tunnin päivässä 80- 85% sydämen maksimipulssista. Mitattiin muutokset kehon kokoomuksessa käyttämällä hydrostattista punnitusta ja tietokonetomografiaa. Perifeerisen kudoksen insuliiniherkkyys mitattiin. Plasman keramidit (C14:0, C16:0,, C18:0, C18:1, C20:0, C24:0,ja C214:1) mitattiin kvantitatiivisesti.
ENGL: METHODS: Twenty-four adults with obesity and normal glucose tolerance (NGT, n = 14) or diabetes (n = 10) were studied before and after a 12-week supervised exercise-training program (5 days/week, 1 h/day, 80-85% of maximum heart rate). Changes in body composition were assessed using hydrostatic weighing and computed tomography. Peripheral tissue insulin sensitivity was assessed by a 40 mU/m2 /min hyperinsulinemic euglycemic clamp. Plasma ceramides (C14:0, C16:0, C18:0, C18:1, C20:0, C24:0, and C24:1) were quantified using electrospray ionization tandem mass spectrometry after separation with HPLC.
TULOKSET: Plasman keramidit olivat samanlaisia lihavuudessa ja normaaliglukoosirasituksessa ja diabeetikoilla, vaikka glukoositoleranssit erosivat. Treenaus alensi merkitsevästi kehon painoa ja lihavuutta ja kohotti insuliiniherkkyyttä molemmissa ryhmissä. Lisäksi kaikilla interventioon osallistuneilla laski plasman keramidien C14:0, C16:0, C18:1 ja C24:9 pitoisuudet . Alenemat kokonaiskeramideissa ja C14:0 keramideissa korreloi negatiiviseeti insuliiniherkkyyteen. ( herkkyys nousi kun keramidipitoisuudet laskivat)
ENGL: RESULTS: Plasma ceramides were similar for the subjects with obesity and NGT and the subjects with diabetes, despite differences in glucose tolerance. Exercise significantly reduced body weight and adiposity and increased peripheral insulin sensitivity in both groups (P < 0.05). In addition, plasma C14:0, C16:0, C18:1, and C24:0 ceramide levels were reduced in all subjects following the intervention (P < 0.05). Decreases in total (r = -0.51, P = 0.02) and C14:0 (r = -0.56, P = 0.009) ceramide were negatively correlated with the increase in insulin sensitivity.
YHTEENVETO: Keramidit linkkiytyvät treenauksen aikaansaamaan paranemiseen insuliiniherkkyydessä. Plasman C14:0 keramidi saattaa olla spesifinen kohdekin tutkittaessa lipideihin korreloivaa insuliiniresistenssiä lihavuudessa ja 2-tyypin diabeteksessa.
CONCLUSIONS: Ceramides are linked to exercise training-induced improvements in insulin sensitivity, and plasma C14:0 ceramide may provide a specific target for investigating lipid-related insulin resistance in obesity and T2D.
